Psychiatric Mental Health Nurse Practitioner (PMHNP) – Medication Management

Position Summary:
The PMHNP will specialize in outpatient psychiatric medication management. Operating in a collaborative and supportive environment, the practitioner will assess, diagnose, prescribe, and monitor medication regimens for patients experiencing mental health disorders. This role requires regular clinical consultations with the Medical Director to ensure evidence-based, holistic patient care.
Key Responsibilities:
Patient Assessment: Conduct comprehensive psychiatric evaluations, establish clinical diagnoses, and develop individualized, evidence-based treatment plans.
Medication Management: Prescribe, monitor, and adjust psychotropic medications to safely and effectively manage symptoms and minimize adverse side effects.
Ongoing Monitoring: Conduct routine follow-up appointments to evaluate medication efficacy, request relevant laboratory tests, and track patient progress.
Patient Education: Educate patients and their families regarding psychiatric conditions, treatment options, potential side effects, and expected recovery outcomes.
Care Coordination: Collaborate actively with the Medical Director, staff therapists, and outside primary care providers to ensure integrated, holistic care.
Clinical Documentation: Maintain accurate, timely, and compliant medical records for all patient encounters, assessments, and billing.
Professional Standards: Adhere strictly to the state Nurse Practice Act, facility policies, and HIPAA guidelines regarding patient confidentiality.
Qualifications & Requirements:
Education: Master’s degree (MSN or DNP) from an accredited Psychiatric Mental Health Nurse Practitioner program.
Licensure: Active, unrestricted license as a Nurse Practitioner in the state.
Certification: Board certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) by the ANCC or AANP.
Authority: Active and current DEA and state-controlled substance registrations.
Experience: Preference for prior outpatient mental health experience, specifically in psychiatric evaluation and medication management.
Skills: Strong diagnostic skills, proficiency with Electronic Health Record (EHR) systems, and excellent interpersonal communication.
