Lighthouse Therapeutic Services is committed to reducing financial barriers to care through a
Sliding Fee Scale (SFS) based on income and household size.

• Discounted rates are available for eligible clients

• Services are available regardless of insurance status

• Payment options include self-pay, Medicaid, and select insurance plans

Lighthouse Therapeutic Services determines eligibility for discounted services based on
household income and size using Federal Poverty Level (FPL) guidelines. Acceptable
documentation includes recent tax returns, pay stubs, benefit statements, or other income
verification. In cases where documentation is not readily available, self-attestation of income is
accepted to ensure access to care. No individual will be denied services due to the inability to
provide documentation or the inability to pay.

We believe behavioral health care is essential. Our team works with each client to identify
affordable options and ensure access to needed services in a supportive and confidential
environment.

Cancellations or missed appointments without 24-hour notice will be subject to a full fee charge. Please note, that insurance companies do not pay charges for missed appointments.
